I would add, teacher/staff perception is more powerful, too. I would give this survey to staff as well. Just change some wording to reflect their context. -

When I was in the classroom it was a simple agree or disagree— for the purpose of feedback on my teaching. As a principal we roll out a school wide student survey that is on a Likert scale—both for the purposes of providing individualized supports and improving our programs.

I love this idea…does anyone have an advice for giving a survey to elementary students, particularly K-3? I see a need to engage student voices in these grades but wonder how I can administer surveys with validity.

Usually they are given orally and for the really young students have pics with smiley or frowning faces :)

Consider gathering just 3-5 pieces of feedback from students grades K-3. Rather than using questions, use pictures or icons that represent the feedback being collected. Explain to the students what the icons represent, have a neutral person give the survey!
